Thank you for your interest in joining LifePlans’ network of field assessors. LifePlans has been in business for almost 20 years and completes health assessments for insurance and health care companies. During our company’s history we have successfully completed over 2 million face to face assessments in insured's homes and our field network has played a large role in our success. Traditionally requests for interviews originate from the insurance company or health care company as a result of a claim or as a way to identify risk in insuring an individual.

Your relationship with LifePlans would be as an Independent Contractor who has signed on to complete assessments on an on-call basis and it is not an employment contract.

Assessment lengths vary dependent on the need of the client but may require capturing medical history, Activities of Daily Living (ADL), Instrumental Activities of Daily Living (IADL), cognitive exercises, height, weight and blood pressure or a facility profile. Assessments can take anywhere from less than one hour to over two hours to complete dependent on what is required by the client. LifePlans never requires lab work or fluid sample collection by our field assessors. LifePlans reimburses on a flat fee schedule based on the assessment being completed.

LifePlans, Inc. subcontracts with agencies and individual Registered Nurses who have at least three years of recent, hands-on geriatric experience to complete our assessments. Members of our field network are Independent Contractors who determine the territory they will cover and have the right to accept or decline a case. The number of assessments each assessor receives fluctuates; therefore most use the assessments as a supplement to their income.

LifePlans, Inc. will send the assessor referrals within that area. The assessor is responsible for calling the client to make the appointment; therefore the hours are very flexible.
