On-Going Care Management / Care Coordination

LifePlans’ Care Management and Care Coordination is an integrated, multi-disciplinary approach. We utilize multi-factorial risk assessments to determine the insured’s level of loss and target areas of concern. We work with the insured, family, providers, physicians and other designated individuals to develop plans of care designed to return the insured to independence (or their prior level of functioning) whenever possible. In cases where the insured’s condition is chronic or deteriorating, the care plan is evaluated and changed as needed to assure that the services provided are appropriate to intensity, duration and service type over time. Service plans are structured to reflect the needs of each client. The goals and plans are centered around the insured’s functional, cognitive and medical status, as well as their personal needs and goals, available family support, financial considerations and eventual discharge plan.

Our Care Coordinator’s role is multi-faceted and includes:
  • Oversight of the provider team
  • Liaison between the insurer, the insured and the provider
  • Educator of the insured or responsible party concerning available, affordable and appropriate local services
  • Empowerment of the insured or family member to seek help from their physician in areas that require such intervention
  • Facilitate the insured’s rehabilitation to the optimal level of function
