The LifePlans Field Network is the strongest across North America, with more than 7,000 Registered Nurses, and growing. We provide care coordination and functional, face-to-face assessments to evaluate cognitive status, measure ADL and IADL dependencies, and make informed judgements on members' ability for self-care and independence, based upon their living environments and overall health.
We're always eager to speak with Registered Nurses with homecare or care-planning experience, and experience with geriatric patients.
We also offer opportunities for Licensed Practical Nurses and non-clinical professionals with industry experience for non-clinical telephonic health risk assessments and phone history interviews.
