The LifePlans National Clinical Network is central to all the services and programs we offer. Through the Network, LifePlans provides care coordination services including face-to-face assessments to evaluate cognitive status, measure ADL and IADL dependencies, and make informed judgments on members' ability for self-care and independence, based upon their living environments and overall health.
The Network includes over 4,000 nurses and is growing across the US, Canada and Puerto Rico.
We're always eager to speak with Registered Nurses with homecare or care-planning experience, and experience with adult and geriatric patients.
LifePlans also offers opportunities for Licensed Practical Nurses and non-clinical professionals with industry experience for non-clinical telephonic health risk assessments and phone history interviews.
