LifePlans' Home Safety Program is designed to:
- Provide individuals with a customized, comprehensive, in-home assessment focused on environmental and home safety as well as a personal health, functional, and cognitive evaluation;
- Make specific suggestions on how to make their home environment safer to assure maximum independence;
- Develop an action plan designed to reduce environmental and fundamental risk factors;
- Supply information regarding health and wellness;
- Help direct or obtain for individuals the services they need to make their home safer and to reduce their personal risk of falls; and
- Follow-up to assure compliance with the action plan.
The program, which can be customized to meet the needs of your organization, helps chronically impaired persons and their caregivers manage their home environments and healthcare needs by linking identified risk factors with specific solutions regarding home safety, health, and wellness.
Our expert network of nurses and geriatric case managers use state-of-the-art assessment methods for problem identification and solution-development. At the home office, we supplement efforts in the field by helping to identify resources to meet those needs, provide referrals to identified resources, and follow up to ensure that each individual's safety needs have been met.
